When consenting to LASIK it is important that I understand the following:

• That LASIK is an operation that consists of using a microkeratome to create a disk or flap on the front surface of the eye (cornea) and an excimer laser to remove microscopic layers of tissue to reduce or eliminate myopia.

• That the results of the operation cannot always be predicted, nor can safety always be guaranteed. To achieve satisfactory vision after surgery, corrective glasses or contact lenses may be neccesary.

• That alternative treatments, such as radial keratotomy and PRK, have been explained.

•That complications from the procedure, such as delayed surface healing, infection, under or over-correction, corneal surface irregularities, excessive corneal haze, regression (especially in patients with greater than -6.00 diopters of myopia), halo effect, decentration, and decreased vision, can occur.



Retreatment may be necessary to attempt to achieve satisfactory uncorrected vision. As with any eye surgery, there are remote risks, such as complete loss of vision.

• That because LASIK is reducing myopia, reading glasses may be needed sooner than would be if the patient were to remain nearsighted.

• The patient is expected to return for scheduled follow-up visits: one (1) day, one (1) month, one (1) week, and twelve (12 ) months after surgery. Additional visits may be necessary.

• If there is a delay between surgery on the first and second eye, there will be a difference in refraction in the two eyes as a result. This may inconvenience the patient or cause visual discomfort.

• Fee will be charged for this operation, including postoperative care for one year. There will be additional charges for medications, postoperative visits after one year, and if required, contact lenses and spectacles. Insurance does not usually cover LASIK.

• This is an elective procedure.

It is impossible for the doctor to inform you of every possible complication that may occur, so please be sure the doctor has answered questions to your satisfaction.
